Yvonne J.F.M. Jansen, PhD. is Program manager for Robuust since 2016, where she now manages a regional program that aims to achieve seamless oncological care between hospital care and primary care networks. Within this program a) local primary care networks are guided how best to form and professionalise multidisciplinary primary care networks for oncological (after)care; b) these networks collaborate in regional knowledge transfer and c) partnership with relevant national stakeholders is formed to establish structural implementation of seamless oncological care.
